1.  Title of work / article of jewelry.

2.  How many you wish to purchase of each article.

3.  For photos: choose: 8x10 - $25 each copy - includes tax and shipping.

                                    11x14 - $40 each copy - includes tax and shipping.

Greeting cards with envelopes - $3 each / 4 for $10

5. For jewelry: price of piece includes tax and shipping. (jewelry prices vary)

6. Your NAME and ADDRESS for shipping purposes and a daytime phone number.
